A short history of anti-rheumatic therapy VIII. The immunosuppressants

چکیده
If pathological anatomy very much center stage during the nineteenth century, with bacteriology gaining greater attention in the second half of that century, it was immunology, perhaps together with endocrinology, which most left its mark on the twentieth century. Although it had been conceived as a specialization in the last decades of the previous century, it is in the twentieth century, and above all from the 1950s onwards, that research in immunology has seen an expansion without precedent in the study of disease and therapies. It was obvious that this research would involve rheumatology and we could even say that this is the branch of medicine which has been most influenced by the advances in immunology. In fact, the interpretation of rheumatic diseases as being “allergic” was first proposed by Fritz Klinge (1), while the 1940s saw two discoveries which opened the way for immunological diagnosis of rheumatic diseases: that of the rheumatoid factor (2, 3) and that of the LE phenomenon (4). Immunoactive therapy had, however, already been started some time before, even though in the beginning it was used to respond to infection and, therefore, to boost the immune response and not suppress it. The anti-smallpox vaccine, introduced by Edward Jenner in 1796 and the anti-diptheria serum, introduced by Emil Adolf von Behring in 1892, can be considered two of the greatest achievements in medicine. (6) It was only in the 1940s the study began of drugs capable orf depressing or suppressing the immune response. However, even in this case these drugs, at least in the beginning, were not intended so much

Rheumatic conditions in human immunodeficiency virus infection.
Many rheumatic diseases have been observed in HIV-infected persons. We, therefore, conducted a comprehensive literature search in order to review the prevalence, presentation and pathogenesis of rheumatic manifestations in HIV-infected subjects. Articular conditions (arthralgia, arthritis and SpAs) are either caused by the HIV infection itself, triggered by adaptive changes in the immune system...
متن کامل[A short history of anti-rheumatic therapy. I. An introduction on traditional and drug treatments].
The origins of anti-rheumatic therapy are very old and mainly related to the use of traditional, sometimes extravagant, treatments, as a part of folk medicine. Spa therapy has long been used for the treatment of rheumatic diseases, as well as, in later times, physical treatments, including electrotherapy. Drug treatment has developed beginning from substances of vegetable origin, such as willow...

Background/Objective: To clarify the state of different immunologic factors in patients with acute rheumatic fever (ARF) and rheumatic heart disease (RHD) compared to healthy individuals.Patients and Methods: Patients with ARF (#21), patients with RHD six months after the onset of ARF (#19), and healthy children from the same age group with normal physical examination and no history of pharyngi...
متن کاملExperimental reactivation of subsiding rheumatic fever.
When anti-inflammatory therapy is reduced or discontinued in patients with acute rheumatic fever, a flare-up of disease activity frequently occurs, manifested by clinical abnormalities or by changes only in laboratory tests. The nature of this rebound phenomenon was recently analyzed in a retrospective survey of 265 patients with acute rheumatic fever (1, 2). The results indicated that most cli...
متن کامل[A short history of anti-rheumatic therapy--VII. Biological agents].
The introduction of biological agents has been a major turning-point in the treatment of rheumatic diseases, particularly in rheumatoid arthritis. This review describes the principle milestones that have led, through the knowledge of the structure and functions of nucleic acids, to the development of production techniques of the three major families of biological agents: proteins, monoclonal an...
